Job Description

About the Role

The Primary Mental Health Clinician & Care Coordinator delivers culturally safe, evidence‑based primary mental health services to Aboriginal and Torres Strait Islander Community residing on Dja Dja Wurrung Country. 

The role delivers both funded streams: Psychological Therapy Services (PTS) for Aboriginal and Torres Strait Islander people, and Primary Care Mental Health Clinical Care Coordination (PMHCCC) for clients with severe and complex mental illness managed in primary care. The position focuses on therapeutic intervention, care coordination, service integration, and client‑centred recovery‑oriented practice.

Key Responsibilities 

  • Deliver culturally safe psychological therapy services to Aboriginal and Torres Strait Islander clients in line with Stepped Care principles.
  • Provide primary care mental health clinical care coordination for clients with severe and complex mental illness.
  • Conduct comprehensive psychosocial and risk assessments, including suicide risk assessment and safety planning.
  • Develop, review, and implement individualised care plans in collaboration with clients, families, GPs, and other providers.
  • Coordinate referrals, shared care arrangements, and multidisciplinary case reviews.
  • Provide brief, medium‑term, and focused psychological interventions consistent with evidence‑based practice.
  • Maintain active engagement and follow‑up of clients on waitlists and in care to meet Murray PHN access and timeliness requirements.
  • Ensure warm referrals and supported transitions of care, including discharge planning.
  • Participate in clinical supervision, peer review, and professional development.
  • Maintain accurate and timely PMHC‑MDS data entry and reporting through approved systems (e.g. FIXUS/CIMS).
  • Contribute to service integration, Health Pathways, and system improvement activities.
  • Provide de‑identified case studies, performance information, and reports as required under the agreement.

Desired Skills and Experience

About You

  • Tertiary qualification in Social Work, Psychology, Counselling, Mental Health Nursing, or relevant allied health discipline.
  • Current registration with relevant professional body (where applicable).
  • Demonstrated experience delivering mental health interventions in primary care or community settings.
  • Strong understanding of trauma‑informed and culturally safe practice.
  • Demonstrated ability to undertake care coordination, risk assessment, and service integration.
  • Experience with clinical documentation and data reporting systems.
  • Ability to work autonomously and as part of a multidisciplinary team.

Education, Training and/or Competencies.

Mandatory

  • Relevant tertiary qualification and professional registration.

Preferred/ desired 

  • Previous experience working with Aboriginal and/or Torres Strait Islander community members or in an Aboriginal organisation (preferred)
